Okuno Pain Procedure (Japan) - C-TAME Standard Transcatheter Arterial Micro-Embolization
Areas of long-term pain are often accompanied by abnormally proliferating microvessels and nerves, which are sources of pain that pain relievers and rehabilitation find difficult to treat.
C-TAME was developed by Dr. Yuji Okuno (Okuno Clinic, Japan), and Xi Clinic has been authorized with its Taiwan trademark to perform it: Under the guidance of the GE OEC 3D mobile C-arm angiography system with cone-beam CT function, microcatheters are used to perform selective embolization of abnormal small arteries in the target pain area, reducing chronic inflammation and the source of pain. This is performed by a radiologist or neuroradiologist in our angiography operating room. The wound is about the size of a pinhole at the wrist. In most cases, local anesthesia is used and hospitalization is not required, followed by outpatient follow-up.
It is suitable for deep pain and recurrent chronic pain such as degenerative arthritis, frozen shoulder, tennis elbow, plantar fasciitis, as well as cases where medication, rehabilitation, or injection therapies have shown limited effectiveness; eligibility must be determined by a physician after consultation, physical examination, and imaging evaluation.
Japanese Okuno Pain Relief Method - s-TAME Simple Transcatheter Microembolization (Ultrasound-Guided, Known in Japan as "Intra-arterial Infusion Therapy")
s-TAME is a simplified approach to microvascular embolization, suitable for superficial pain in the distal extremities, known as "transarterial microembolization" in Japan.
Under ultrasound guidance, the physician uses a fine needle to inject the agent into the abnormal microvascular area of the painful site, without using catheters, contrast agents, or angiography equipment. The process is similar to a standard ultrasound-guided injection, and patients can resume normal activities on the same day after treatment. It is suitable for pain that is superficial with a well-defined area, such as tennis elbow, de Quervain's tenosynovitis, plantar fasciitis, or chronic inflammation at superficial tendon insertion sites. Xi Clinic primarily offers catheter-based C-TAME, with s-TAME as an option for superficial pain. The choice between the two is not based on price, but is determined by the physician after assessing the depth, area, and vascular structure of the pain: s-TAME is sufficient for superficial, well-defined pain, whereas deep or complex painful areas require the catheter and cone-beam CT localization capabilities of C-TAME.
Erectile Dysfunction Treatment (Erectile Dysfunction Treatment)
Venous Occlusion Treatment (Erectile Dysfunction Venous Occlusion Procedure): For cases where excessive venous blood flow out of the penis makes it difficult to maintain an erection, this procedure selectively blocks abnormal leaking veins to improve corporal filling and erection maintenance. It is a minimally invasive treatment with a short procedure time and can be evaluated and followed up on an outpatient basis.
Arterial Revascularization Surgery (Arterial Revascularization for Erectile Dysfunction): For erectile difficulties caused by insufficient arterial inflow to the penis, this procedure uses arterial bypass, grafting, or reconstructive surgery to improve blood flow to the corpora cavernosa and enhance erectile function. It is most often used in younger patients or those with blood vessel disruption due to trauma, and it is a highly specialized minimally invasive vascular surgery.
Prostatic Artery Embolization (PAE)
Frequent urination, nocturia, and weak urination caused by an enlarged prostate (benign prostatic hyperplasia) are traditionally treated with medication or transurethral surgery. PAE (Prostatic Artery Embolization) offers another option: an intervention specialist inserts a catheter through the artery to selectively block the blood flow supplying the prostate, shrinking the size of the gland and relieving symptoms. This process does not involve the urethra or the removal of tissue. At Xi Clinic, a cardiovascular surgery specialist conducts evaluations using extremity vascular ultrasound, selecting treatment options based on the location of reflux and its severity. Options include medical-grade compression stockings, laser, and minimally invasive vein glue closure surgery. Treatments are completed at our clinic with a short recovery period.
