What is the difference between amniotic membrane and TAME? How do they differ from PRP and hyaluronic acid? Comparison of the four treatment mechanisms

PRP, Hyaluronic Acid, Amniotic Membrane, and TAME: These Four Treatments Are Actually Divided into Two Categories: Tissue Repair and Cutting Off Nutrients

In the pain clinic, these four treatments are most frequently mentioned together. We can categorize them first, and then compare them, so as not to confuse their therapeutic mechanisms.

PRP, hyaluronic acid, and amniotic membrane belong to the same treatment type: they are all injected, delivering materials directly into the painful tissue for repair. The difference lies in the formulation of the injectant.

PRP is platelet-rich plasma containing growth factors separated from one's own blood; hyaluronic acid is a medium for lubrication and cushioning; amniotic membrane-derived preparations are obtained from donated placental tissue, processed to retain growth factors and extracellular matrix.

TAME (Transcatheter Arterial Microembolization), on the other hand, is a different type of treatment. While the aforementioned three focus on repair, TAME uses a catheter to locate abnormal neovascularization associated with chronic pain and embolizes these abnormal microvessels with microspheres, stopping the supply of nutrients to the affected area.

What Is the Difference Between PRP and Hyaluronic Acid?

The mechanism of PRP is tissue repair, using growth factors to stimulate tissue and achieve repair functions; the function of hyaluronic acid is lubrication and cushioning, primarily for symptom relief. What Is Amniotic Membrane? What Is the Difference Between Amniotic Membrane and PRP?

The biggest difference lies in the source. PRP is sourced from one's own blood, whereas amniotic membrane comes from screened, donated placental tissue, processed to retain growth factors and extracellular matrix. One is autologous, and the other is allogeneic.

Current literature mainly focuses on two symptoms.

Knee Osteoarthritis: In a multicenter randomized controlled trial of 200 people, the amniotic suspension injection group showed better improvement in pain and function at three and six months than the hyaluronic acid and saline control groups, with 12-month extended follow-up showing sustained improvement.

Plantar Fasciitis: In a randomized controlled trial of 145 people, a micronized injectable preparation was significantly superior to placebo in pain and functional indexes at three months.

So, what is the relationship between amniotic membrane and PRP? The treatment positioning is quite clear: amniotic membrane is one of the advanced options that can be evaluated when PRP treatment shows limited effectiveness. The mechanisms and sources of the two are different; suitability is evaluated by a physician.

What Is the Difference Between Amniotic Membrane and TAME?

Amniotic membrane promotes tissue regeneration and repair, while TAME stops the transport of nutrients.

Amniotic membrane is a tissue regeneration and repair injection, providing the necessary conditions for damaged tissues to heal. The concept of TAME, conversely, considers that chronic pain is often related to abnormal neovascularization (abnormal microvessels). These blood vessels grow along with nerves, and inflammation and pain are continuously supplied by the bloodstream. TAME uses a catheter to embolize these abnormal vessels, blocking this pathway of blood flow and nerve-induced pain.

"Which treatment, amniotic membrane or TAME, is more suitable for me?" In fact, this question has a very different conceptual basis. What we should think about is: where does your pain mainly come from? After an ultrasound scan, if the condition of tissue repair is found to be poor, regenerative and repair-type injections can be considered; if imaging reveals abnormal neovascularization, the pain has lasted for more than three months, and conservative treatment or prolotherapy has limited effects, TAME is an evaluation option that can be discussed next. Both issues may also coexist, which is precisely why we insist on a comprehensive evaluation.

Additionally, it is important to note that TAME addresses the mechanism of pain. If you experience numbness or weakness, these are neurological symptoms and do not fall within the indications for TAME. If such symptoms appear, a consultation with a neurosurgeon can be arranged to identify the cause.

Can They Be Used Together in Treatment? How Is the Order Arranged?

There is a sequence to treatment: we can start with lifestyle adjustments, rehabilitation, medication, and injection therapies. If conservative treatments have limited effects, further interventional options are then evaluated.

Frequently Asked Questions

If PRP didn't work, is the next step amniotic membrane or TAME? Don't rush to make a choice; let's go back to "why it didn't work." If your body's own repair capacity is insufficient, having a doctor adjust the regenerative injection content is a direction worth considering. However, if examinations reveal that the source of pain is actually abnormal blood vessels, injecting more regenerative agents will not achieve the desired effect. Is amniotic membrane more effective than PRP? The sources and mechanisms of the two injection methods are different, and the circumstances studied in the literature also vary, as do individual cases.
